    To ensure we provide the highest quality care for your child, we kindly ask that you complete the Pediatric Medical History and Signature Forms electronically on our Patient Dashboard. These forms are required and help us maintain the most accurate and up-to-date information. You may access the Patient Dashboard Login page by clicking the button provided.
     
    First time users will be emailed an invitation to login with a temporary password. You will need to change this password at first login. If you forget your password, select the "forgot password" button. A reset email will be sent to the email you used to register your child.
